Mental Health Practitioner
Mental health practitioners provide wellbeing-focused psychologically informed interventions and coordinate care plans for adults with severe mental health problems. Based in the community, they have an important role in supporting adults of all ages to help them live fulfilling lives. Health Coach
Health and wellbeing coaches support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them.
Social Prescriber
Social prescribing link workers connect people with local community activities and services that can help improve their health and wellbeing.
Community Midwives
The community midwives work with doctors to provide care for mothers before and after delivery and care for the baby for the first ten days of life. They can be contacted on 020 8296 2990/2761 during the daytime and via the labour ward at St Helier Hospital at night. There is a “talk to midwives 24/7” service that is reachable on 0300 123 5473.

Community Nurses
The District Nurses provide nursing treatment and advice to the housebound and patients recently discharged from hospital. The District Nurses can be contacted during the daytime on 020 8915 6480 and at night on 020 8915 6200.
Health Visitors
Your health visitor is a qualified general nurse with further training in Community Health and all aspects of family health and well-being. Health visitors routinely visit families with children under the age of five years. They provide health assessments, information, advice and support to all age groups, particularly at times of physical or emotional stress. Many have additional qualifications and experience in different aspects of nursing, e.g. midwifery, sick children’s nursing and mental health. Nursery Nurse
he nursery nurse works alongside the health visiting team to promote the health and well-being of individuals and families within the practice. As well as the NNEB qualification many nursery nurses have additional training and experience including therapeutic play. They provide advice and support in the areas of nutrition, behaviour and sleep management, play and stimulation and parenting skills.
